I answered a test question incorrectly, but the number of questions in my Challenge Bank did not increase. Why?

The Challenge Bank does not contain duplicate questions. A question is added to the Challenge Bank only once no matter how many times you answer it incorrectly. However, if you remove a question from the Challenge Bank and later miss that question again, we'll add it back to the Challenge Bank so you can review it later.
